The Career Pathways Training (CPT) Program prepares individuals to start or advance careers in 13 high-demand health, behavioral health, and social care roles—strengthening New York State’s health, behavioral health, and social care workforce for today and tomorrow.

become a qualified service commitment site

Each participant commits to a three-year service agreement with a qualified NYS Medicaid-enrolled provider or social care network site serving at least 30% Medicaid and/or uninsured individuals.

To become a qualified service commitment site, you must meet and attest to any one of the following criteria:

  • Be an active Medicaid-enrolled provider with a patient population that is at 30% Medicaid-reliant or uninsured members or,
  • Be a Social Care Network (SCN) lead entity or,
  • Be a not-for-profit community-based organization that is contracted with an SCN and is providing health-related social needs (HRSN) screening, referrals, and/or services or,
  • Be a contracted not-for-profit provider partner of an SCN and provide HRSN screening, referrals, and/or services (e.g., health or behavioral health provider).

Note: Medicaid members include individuals enrolled in NYS Medicaid fee-for-service, managed care, or dual-enrolled.

FOR PROVIDER PARTNERS

Completed attestations, with the qualification table if applicable, can be submitted to the CPT Partner Team at cptpartners@flpps.org.

There is no established cutoff date for completing and submitting the attestation form to become a qualified service commitment site.

No. If your organization has multiple sites that meet the 30% threshold, each site must be listed separately even if they have the same Medicaid ID. Submitting the qualification table with each site listed, along with the attestation form, allows for one signature to cover multiple service commitment sites.

No, completed attestations will be shared amongst the WIOs.

At this time, the participant is required to pay back the entirety. We continue to advocate for this with the NYS DOH sponsoring this grant

FOR EDUCATIONAL/TRAINING PARTNERS

Yes. Expenses not covered by the CPT Program include but are not limited to:

  • Room and board
  • Living expenses
  • Meals
  • Other incidental expenses
  • Parking
  • Transportation
  • Technology such as laptops or tablets
  • Background checks
  • Vaccines
  • N95 fit testing
  • CPR certification
  • Drug testing
  • Any other standalone processes that are not part of tuition but may be required for programs and/or clinical placements

No. Students will not need to pay taxes on the funds from FLPPS.

Yes. FLPPS will cover the cost of repeating a course. In addition, the student will receive increased educational case management and tutor support.

No. All participants must complete their education/training programs by the Spring of 2027.

Yes. There is no requirement that the student cannot apply if they have a degree.

Students usually qualify for one or the other. We advise the students to review and decide which grant best meets their needs.

We will pay for the student to repeat the class or classes. The Success Coaches and tutors will be available to help them create a plan for success.

Defer to the education/training institution financial office to best answer this question.

Our goal is to make sure we have local job for all job titles. We currently approximately 90 contracts with local systems. We are in the process of obtaining the contracts from the larger health systems in the area. We will continue to evaluate the participants address and work to connect with the systems in the areas surrounding. Please let us know if there are specific health systems/locations that you would like us to connect with.

All funds paid on your behalf up to that point. The reimbursement will be paid back to FLPPS or the state. We are working through the details. It will not be a requirement of the educational institutions to collect this.

The CPT program funds are “last dollar” funds for tuition and books. If a federal loan is taken out to cover anything the CPT Program funds do not cover, such as living expenses, food etc., this is separate from the expenses that CPT Program funds.

Students are not required to pay taxes.

In the event that your organization needs to return any unearned PELL dollars, you would be able to bill FLPPS for the unearned funds. We would request that you follow the same procedure in terms of documenting it on a Financial Roster Invoice (which we can provide) and providing the support documentation to demonstrate what happened. We would ask as well in the event that the PELL or other funding sources were underestimated that you please let us know that as well. We can discuss these items if they end up occurring but based on timing and if you have another term underway when this is reported we can lump any settle-up from the under/overestimated PELL in with the payment for that term if that lessens the administrative burden for you.

backfill Payments for provider partners

To avoid reduced access to care when a CPT Program participant is in training during their working hours and recognizing the absence requires a temporary or covering worker to perform duties, employers may be eligible for funding to backfill CPT participants. Funding may only be used to backfill positions left vacant by participants in the Career Advancement Pipeline.

Backfill expenses are used to cover the cost of a temporary replacement worker when current employees participating in the CPT Program must be absent to attend CPT training or educational programs. Employers are expected to continue paying CPT participants during their excused absences for training.

Current NYS Department of Health guidelines allow for two (2) days/week for backfill to employers.
