About the CPT Program
Employers
All CPT participants are required to fulfill a three-year service commitment by working for a healthcare provider within New York State.
To qualify as a hiring employer, organizations must meet at least one of the following criteria:

Be a Medicaid-enrolled provider with a patient base that is at least 30% Medicaid-reliant or uninsured

Serve as the lead entity for a Social Care Network (SCN)

Operate as a not-for-profit Community-Based Organization contracted with an SCN and actively provide HRSN screenings, referrals, or services

Be a not-for-profit provider contracted with an SCN, offering health-related or behavioral health HRSN screenings, referrals, or services
You must also fill out an attestation to become a partner.
